Request Account & Data Deletion
Option 1: Delete in-app (fastest)
If you can still log in, you can delete your account and all associated data (notes, patients, templates, macros) yourself, instantly:
- Log in to MD Record AI and go to Settings.
- Scroll to the Danger Zone section.
- Click Delete Account and confirm by typing DELETE.
This permanently deletes your account and data immediately.
Option 2: Submit a request
Can't log in? Submit a request below and we'll delete your account and data within 30 days.

What gets deleted
Your account profile, notes, transcripts, patients, templates, and macros are permanently deleted.
Some data may be retained longer where required by law (e.g. HIPAA minimum retention periods). See our Privacy Policy for details.
